Contact Clear Ketamine Therapy

Reach Clear Ketamine Therapy in Tampa. Request a consultation, ask a question, or plan a visit — with clear guidance on what not to send.

Ways to reach us

The simplest way to start is the consultation request form on this page, or call us at (813) 851-5045.

Planning a visit? Our clinic is at 27724 Cashford Circle, Suite 102, Wesley Chapel, FL 33544, in Pasco County just north of Tampa. Sessions run about two hours door to door and you cannot drive yourself home afterwards, so arrange a ride before you come.

You do not have to travel to talk to us. Consultations and follow-up visits can happen in person at the clinic or by telehealth — tell us which you would prefer and we will book that. Treatment sessions themselves always take place at the clinic, with monitoring.

What the consultation form asks

The form is short by design. It asks for your name, how you prefer to be contacted, the best times to reach you, and a brief general reason for reaching out — something like “questions about candidacy” or “cost questions” is plenty.

  • Your name and preferred contact method
  • The best times and days to reach you
  • A brief, general reason for contacting us
  • Anything you want us to know about scheduling constraints

Please do not send medical details through the form

This form is for starting a conversation, not for transmitting health information. Please do not include your diagnosis, medication lists, medical records, or detailed symptoms in your message — one general sentence about why you are reaching out is all we need at this stage.

Your full history matters, and it will be collected properly: through the clinical evaluation, via appropriate channels, by the people providing your care.

If you need urgent help

Our contact form, email, and phone lines are not monitored for emergencies, and ketamine therapy is never an emergency treatment. If you are in crisis or having thoughts of self-harm, call or text 988 to reach the Suicide & Crisis Lifeline, or call 911.

Please do not use the form to seek urgent help — a form response may not come quickly enough. The 988 line is staffed at all hours, every day.

What happens after you reach out

A member of our team reviews every message during business hours and responds as quickly as we can. We will answer your questions as best we can and, if you wish, schedule an initial evaluation — in person or by telehealth, your choice.

Reaching out creates no obligation. You will not be added to a pressure campaign, and your information is handled according to our privacy policy, which is written in the same plain language as the rest of this site.

In person or by telehealth

You can be seen both ways. Consultations, planning conversations, and follow-up visits happen in person at our Wesley Chapel clinic or by telehealth — whichever works better for you.

Ketamine itself is always administered in person, at the clinic, with a clinician monitoring you throughout. We do not prescribe ketamine for use at home.

Telehealth visits require you to be physically located in Florida at the time of the visit.

How telehealth works here

  • Telehealth is available to patients who are physically located in Florida, and the clinicians providing that care hold active Florida licenses.
  • Your location is confirmed at the start of every telehealth visit. If you are not in Florida, the visit does not go ahead.
  • Ketamine is never prescribed for you to take at home. It is administered only at our clinic, in person, with a clinician monitoring you throughout — a telehealth visit does not change that.
  • You are seen in person before any treatment session, because every session happens at the clinic.
  • A telehealth visit never guarantees that treatment will be offered, any more than an in-person evaluation does.
  • Telehealth is not for emergencies. If you are in crisis or having thoughts of self-harm, call or text 988, or call 911.

Common questions

Both are available. Your evaluation, planning conversations, and follow-up check-ins can happen in person at our Wesley Chapel clinic or by telehealth — whichever suits you, and you can switch between them. Telehealth visits require you to be physically located in Florida at the time of the visit. Treatment itself is the exception: ketamine is always administered in person, at the clinic, with a clinician monitoring you throughout. We do not prescribe ketamine for use at home.
